Prior to your first appointment, you’ll receive and must fill out a brief confidential health history form. This form must be updated once a year or any time there are significant changes to your health history (surgery, major illness, etc.). The information in your file can only be released with your consent or as required by law. In order to best serve you, I may discuss your case with other healthcare practitioners.
Scheduled appointment times are reserved especially for you. I have a 24-hour cancellation policy, and for this reason I’ll ask to keep a credit card securely on file. If you cancel or reschedule your appointment with less than 24 hours' notice you will be charged the full treatment cost.
Refusal of Service:
For safety purposes, I reserve the right to refuse or discontinue treatment at any time. Reasons may include, but are not limited to, the following:
derogatory or bigoted comments
illness or infectious conditions
contagious or unidentified skin conditions (for example, rashes, warts, mysterious bug bites)
Illicit or sexually suggestive remarks, advances, or intentional physical reactions meant to provoke a response from me are NOT tolerated. I'll immediately end the session, you’ll be charged the full treatment cost, and banned from my practice.
If you are sick, please reschedule your appointment. If you develop a skin condition, please clear it with your doctor before receiving massage. Thank you for keeping my practice safe.
You will always be discreetly and securely draped. North Carolina licensed massage therapists are required to provide draping in a manner that ensures your safety, comfort and privacy. Areas that are customarily draped during treatment are the gluteal and genital areas for male and female clients, and breast area for female clients.
